K’ima:w Medical Center welcomes you to their Patient Centered Medical Home Community Event, today from 10:00 am to 6:00 pm at the Hoopa Neighborhood Facilities!
At this event you will gain knowledge on how K’ima:w Medical Center is working as a PCMH to build better relationships between patients and their clinical care team.
You can meet your team and get a free meal ticket! Meet ALL the teams and get a gift card! If travelling from out of the area, bring proof of address for a gift certificate.
