Lifeline Phones

The Hupa Family Resource Center will assist the community in obtaining a Lifeline Phone, today June 24th from 9:30 am to 5:00 pm in the Parking Lot in front of the Hoopa Shopping Center in Hoopa.
You need to bring Proof of Identity: A California Driver’s License or ID, Tribal ID or a US Passport. The – ID must be legible, cannot be Expired and must have an expiration date. You must provide proof of physical address: Like a California Driver’s License or ID, Tribal ID, Passport to Services, utility bill, DMV paperwork or any other form that shows your Physical Address, not Post Office box number;
And provide Proof of Income: Current CalFresh benefits card, Tribal Commodities verification, Tribal TANF verification, Utility bill, Passport to Services, SSI Award Letter, MediCal/Medicare card or current employment pay check stub. Verification must be current and legible.
